Bottom line: Fish oil lowers triglycerides (a blood fat), but no marketed benefit is established at label doses, so if you take it, use a modest dose of fresh, certified oil.

What’s New This Month

  • Warning letter: In January 2026 FDA warned Supersmart USA that its Super DHA fish oil product was marketed with disease claims. 1
  • Adverse-event reports: FDA’s database holds 73 self-reported reports naming a fish oil product between October 2024 and July 2026. These are unverified and do not show the product caused the event. 2
  • Heart rhythm: The 2026 review of 35 trials still links more atrial fibrillation (an irregular heartbeat) to doses above 1,500 mg a day in people at high heart risk. An omega-3 trade group helped pay for it. 3
  • Corrections: A heart-failure review we earlier said did not exist does exist: only high-dose DHA-rich oil improved heart pumping in it. The Health Canada fish oil page is now dated 25 September 2026. 4 5

Overview

Fish oil supplies two omega-3 fats, EPA and DHA, taken from oily fish. It is one of the most studied supplements in the world.

It is sold as softgels and liquids. A “1,000 mg fish oil” softgel often has only about 300 mg of EPA plus DHA, so check the combined figure.

The largest review, from 2020, pooled 86 trials in 162,796 people and found little or no effect on heart events. 6

Labels claim it supports the heart, brain, joints, eyes, mood, immunity, liver and skin.

How We Researched It

We rate each marketing claim with a modified version of GRADE, the system medical researchers use to say how sure we can be (i.e., high, moderate, low or very low), and check each trial with the Cochrane risk-of-bias tool, which asks whether a study’s design could skew its results. For fish oil we used about 35 evidence records, mostly from October 2022 to October 2026, plus older landmark trials. We also checked FDA, Health Canada, NIH, EFSA, Prop 65, Japan, Korea, Cochrane and ConsumerLab.

Research NotesTop three benefits

Blood Pressure GRADE: Low

A review of 71 trials found 2 to 3 g a day lowered top blood pressure by about 2.6 points. 7

Bias check: The review is from 2022, before our main window, and the drop is small.

Mood GRADE: Very Low

A review of 10 trials in 1,426 people found EPA-rich oil eased depression scores a little. 8

Bias check: Trial results disagreed sharply, and negative trials may be missing.

Joints GRADE: Very Low

A 262-person trial of 2 g a day of krill oil for 24 weeks found no less knee pain than placebo. 9

Bias check: Krill oil is not fish oil. Health Canada allows a rheumatoid arthritis pain claim at 2,800 to 5,000 mg a day, which is a label rule, not a trial result. 5

What Sellers Say vs. What Studies Show

“Protects your heart.”The largest review found little or no effect on heart events. FDA calls the evidence for its heart claims inconsistent and inconclusive. 6 10
“Boosts memory and brain health.”A 2-year trial of 2 g of DHA a day in 365 older adults found no gain in memory or brain size. 11
“More is better.”High-risk people taking over 1,500 mg a day had more atrial fibrillation. At 4 g a day, stomach upset rose from 15% to 25%. 3 12
“Flavored means fresh.”Flavor can hide rancid oil. In one test, 68% of flavored oils were over the rancidity limit, against 13% of plain oils. 13
“Helps dry eyes.”A 2026 review of 27 trials said omega-3 failed to show consistent benefit for dry eye. Korea recognised a dry-eye function in 2015, but that is a label rule, not a trial. 14 15
“Clinically proven.”It is well studied, but only triglyceride lowering has moderate evidence, and that is a lab number.

Trial ParametersIf you try it

WhoNo marketed health benefit is established. Prescription omega-3 is the route for high triglycerides.
Dose250 to 500 mg of EPA and DHA a day for general use, up to about 1 g. 16
CeilingUnder 2 g a day (ConsumerLab); FDA and EFSA cite 5 g. Above 1.5 g, ask first if heart risk is high. 16 17
DurationNo clinical stop rule established. Discard if rancid.
FormEPA and DHA stated per serving, with a certificate of analysis or USP or NSF listing; keep cool.
Ask FirstHeart disease or atrial fibrillation; anticoagulants (blood thinners); surgery; fish allergy.
Source: 16 17 and studies cited.

How to Take It

Pick a planFor general use, 250 to 500 mg of EPA plus DHA a day. Read the EPA plus DHA figure, not the oil weight.
Time of dayNot reported.
With foodTake it with a meal that has some fat. Fat helps your body absorb it.
What to expectTriglycerides may fall at 2 g a day or more. Fishy taste, heartburn, nausea or loose stools can occur. 17
How to takeKeep it cool and stop if it smells rancid. Health Canada says to refrigerate liquid oil after opening. 5

Works With / Don’t Mix With

✓ Works with

A meal with fat. Fat in the meal helps absorption.

Oily fish. Consumer Reports advises eating fatty fish before turning to pills.

Not reported. We found no supplement shown to add to fish oil’s effect.

⚠ Ask first / don’t mix

Blood thinners and surgery. NIH says 2 to 15 g a day may lengthen bleeding time. Warfarin users should have regular clotting (INR) checks. Ask before surgery. 17

Heart disease or atrial fibrillation. Ask your doctor before taking over 1,500 mg a day; high-risk people had more atrial fibrillation. 3

Fish allergy. Allergists advise asking your allergist first. Refining should remove fish protein, but that is not certain. 18

Pregnant or breastfeeding? Our review did not evaluate it, so ask your doctor.

Three formal FDA recalls were still open and one was closed in the past 24 months, plus two warning letters. FDA’s adverse-event database holds 73 reports naming a fish oil product (14 hospitalisations, 6 life-threatening, 3 deaths among the outcomes); these are unverified and do not show the product caused the event. 2 California also lists lead and cadmium notices naming a cod liver oil maker (Lysi); the product type is not confirmed, and they are allegations. 25 We found no Health Canada fish oil recall since October 2024. Our review did not search Australia’s regulator or EU RASFF.

Where to Buy

We compare products side-by-side not just on price, but on whether NSF lists the product as NSF/ANSI 173 Contents Certified (an independent lab confirms the label matches the contents), whether it is NSF Certified for Sport, whether USP lists it as USP Verified, and whether the seller says it is manufactured in the USA. We leave out a brand’s products for this supplement when, in the last 36 months, the FDA took action against that brand over this supplement (a Class I or II recall, a warning letter, an import alert or an advisory letter). We also leave out any brand with an FDA Class II recall or warning letter on any product. We also leave out any product named in a California Prop 65 notice or a non-US recall, along with that brand’s products of the same ingredient and form. Brands with other actions stay listed and are marked in the Recall Screen column. Our goal remains to provide you, the Consumer, with the information and data necessary to make an informed purchasing decision. Prices reflected below are static and subject to change.

Ratings above describe the ingredient’s research, not any product listed below.

Recall Screen: ✓* means the brand’s only actions in the last 36 months are a California Prop 65 notice, a non-US recall or sales action, an FDA Class III supplement recall, or a food recall, each on a different product from the one listed. Prop 65 notices are allegations by private enforcers, not findings. A blank means the brand had another FDA action in that period, such as an import alert or a recall of a different product, or a minor (Class III) recall of this product.
