Bottom line: CoQ10 may lower heart-failure deaths, blood pressure and migraine attacks, but the evidence is low quality, and some labels exceed the 300 mg a day Canada and Australia allow.

What’s New This Month

  • Ubiquinol: In a 2026 trial of 150 healthy adults, ubiquinol raised blood CoQ10 no more than ordinary CoQ10 (ubiquinone) at 200 mg a day. A supplement brand funded the trial. 1
  • Exercise: A 2026 review of 24 studies in 613 athletes found no overall benefit for performance or fitness. 2
  • Adverse events: FDA’s adverse-event system holds 59 reports since October 2024 naming a CoQ10 product: 19 as a suspect, 40 only alongside other products; none lists a death. These are reports to FDA, which cannot show cause or how often harms happen. 3
  • Lawsuit: A lawsuit filed in June 2026 alleges that Qunol Liquid CoQ10’s “superior absorption” claims are false. The case is open; these are allegations only. 4

Overview

Coenzyme Q10 (CoQ10) is a compound your cells use to turn food into energy. It also acts as an antioxidant, which helps protect cells from damage. Your body makes its own, and statin drugs lower the amount in your blood. 5 6

Scientists first isolated CoQ10 in 1957 and worked out its structure in 1958. In the US it is sold as a dietary supplement, not an approved drug. 7

Labels claim it helps the heart, blood pressure, energy, migraine, statin muscle pain, fertility, memory and skin. It comes as ubiquinone or ubiquinol, a form sold as easier to absorb.

How We Researched It

We rate each marketing claim with a modified version of GRADE, the system medical researchers use to say how sure we can be (i.e., high, moderate, low or very low), and check each trial with the Cochrane risk-of-bias tool, which asks whether a study’s design could skew its results. For CoQ10 we appraised 51 records, published mostly from October 2022 to October 2026, plus older landmark trials. We also checked 10 regulators and safety databases, and used two Cochrane reviews.

Research NotesTop three benefits

Heart-Failure Support GRADE: Low

In the Q-SYMBIO trial, 420 people with heart failure took 100 mg three times a day for 2 years; 10% died, versus 18% on a dummy pill. 8 A 2024 review of 32 trials found 36% fewer deaths, but that one trial carries much of the result. 19

Bias check: Q-SYMBIO was funded by CoQ10 makers and has not been repeated. A Cochrane review found no convincing evidence either way. 9

Blood Pressure GRADE: Low

A 45-trial review found the top blood-pressure number fell about 3.4 points. The bottom number did not change. 26

Bias check: Results varied widely between trials, and the effect shrank as more trials were added. A Cochrane review found no meaningful effect. 27

Energy and Fatigue GRADE: Low

A 13-trial review of 1,126 people found fatigue eased a little. 28 A newer 5-trial review found no clear effect on fatigue. 29

Bias check: The effect was below the size most people would notice, and trials mixed very different patient groups.

What Sellers Say vs. What Studies Show

“Clinically proven for heart health.”No trial of CoQ10 alone has tested heart outcomes in healthy adults. The heart-failure result leans on one industry-funded trial, and a Cochrane review found no convincing evidence either way. 8 9
“Ubiquinol works better.”In a 2026 trial of 150 adults, ubiquinol raised blood CoQ10 no more than plain CoQ10. No trial shows better health results. 1
“It fixes statin muscle pain.”Reviews disagree, and an NIH-funded trial found no benefit. 10 11
“Statins drain 40% of your CoQ10.”That figure is blood levels in a 30-person study from 1993. In a 2022 trial, supplements did not raise CoQ10 inside muscle. 6 12
“It sharpens your memory.”A 2026 trial in 111 older adults found no change in memory or mood. 13
“It boosts fertility.”A registered IVF trial of 100 women found more eggs but no difference in pregnancy. 14

Trial ParametersIf you try it

WhoNo marketed health benefit is established for any group. Not a substitute for prescribed heart, blood-pressure or migraine treatment.
DoseHealth Canada: 30 to 300 mg a day; 150 to 300 mg a day for migraine, for at least 3 months. 15
CeilingCanada and Australia: 300 mg a day; Singapore: 150 mg. Some retail labels list up to 400 mg a serving. 15 16 17
DurationJudge benefit at 8 to 12 weeks (3 months for migraine) and stop if none.
FormTake with a fat-containing meal; split doses of 100 mg or more. Ubiquinol was no better than ubiquinone in a direct trial. 1
Ask FirstWarfarin or blood thinners; chemotherapy or radiation; blood-pressure or diabetes drugs; biliary obstruction; pregnancy or breastfeeding; children 15 and under.

How to Take It

Pick a planHealth Canada lists 30 to 300 mg a day, and 150 to 300 mg a day for migraine. Split doses of 100 mg or more into two. 15 5
Time of dayMorning or midday if it disturbs your sleep. Trouble sleeping is reported at 100 mg a day or more. 5
With foodYes. Take it with a meal that has some fat; your body needs fat to absorb it. 5
What to expectYou may not feel it. Judge any benefit at 8 to 12 weeks, or 3 months for migraine, and stop if you see none. 15
How to takeSwallow softgels whole with food. If you buy ubiquinol, choose a listing with an independent quality certificate; ConsumerLab reported 24 Amazon ubiquinol products with little or none inside. 18

Works With / Don’t Mix With

✓ Works with

A meal with some fat. CoQ10 needs dietary fat to be absorbed. 5

Your prescribed heart, blood-pressure or migraine care. It was studied as an add-on, never as a replacement. 19 9

⚠ Ask first / don’t mix

Warfarin and other blood thinners. Health Canada says to ask a practitioner first above 100 mg a day, and Australia and Singapore require the label warning “Do not take while on warfarin therapy without medical advice.” Get an INR blood test after starting or stopping. 15 17

Chemotherapy or radiation. No trial shows it is safe during cancer treatment. Ask your cancer doctor first. 7

Blood-pressure or diabetes drugs. Health Canada says to ask first if you take blood-pressure medication; NCCIH notes a possible interaction with insulin. 15 20

Also ask first if you are pregnant or breastfeeding. 15 Not reported: combinations with other supplements.

Where to Buy

We compare products side-by-side not just on price, but on whether NSF lists the product as NSF/ANSI 173 Contents Certified (an independent lab confirms the label matches the contents), whether it is NSF Certified for Sport, whether USP lists it as USP Verified, and whether the seller says it is manufactured in the USA. We leave out a brand’s products for this supplement when, in the last 36 months, the FDA took action against that brand over this supplement (a Class I or II recall, a warning letter, an import alert or an advisory letter). We also leave out any brand with an FDA Class II recall or warning letter on any product. We also leave out any product named in a California Prop 65 notice or a non-US recall, along with that brand’s products of the same ingredient and form. Brands with other actions stay listed and are marked in the Recall Screen column. Our goal remains to provide you, the Consumer, with the information and data necessary to make an informed purchasing decision. Prices reflected below are static and subject to change.

Recall Screen: ✓* means the brand’s only actions in the last 36 months are a California Prop 65 notice, a non-US recall or sales action, an FDA Class III supplement recall, or a food recall, each on a different product from the one listed. Prop 65 notices are allegations by private enforcers, not findings. A blank means the brand had another FDA action in that period, such as an import alert or a recall of a different product, or a minor (Class III) recall of this product.
